We are looking for an enthusiastic individual with excellent communication and organisational skills to work as a Ward Clerk supporting our End of Life Facilitators and the Community Matrons Virtual Ward.This role is for up to 37 hours per week, fixed term contract for 1 year.
This key role will entail:
Admitting and discharging patients onto our electronic records system, data entry into SystmOne and maintaining various spreadsheets, training in our systems will be provided.
Booking and coordinating a variety of meetings and education sessions, such as the monthly multi-disciplinary team meeting and weekly palliative care meetings.
The role will require you to liaise closely with the Community Matron and End of Life Facilitators teams.
You will have regular telephone contact with Swindon GP practices, Prospect Hospice, the Community Nursing team and other professionals in gathering patient information, maintaining accurate records, and building a professional rapport, locally and nationally.
Producing reports, presentation and display material for education and promotion.
We require you to work flexibly to meet service demands, which may require working across seven days a week, evenings, and Bank Holidays, as required.
