From October 2015 the Commissioning Team within Adult Social Care will hold responsibility for the care management and duty function of the adult learning disability team, including adult transitions. In addition we are looking to expand our Service Manager capacity across the Adult Social Care directorate.
This is an exciting opportunity to be part of a newly forming team, to be innovative, influence and design the further integration of adults and children for those service users with a learning disability and fully implementing the principles of the SEN Reform.
